Pull request overview
Updates the .github/skills/code-review guidance to more strongly require reviewers to consult prior review iterations and avoid duplicating feedback across iterations.
Changes:
- Expands “Existing review comments” guidance to include resolved/outdated/addressed comments and discourage re-posting previously raised feedback.
- Extends the “Don’t pile on” guidance to also discourage repeating the same feedback across review iterations.
