  • Implement KAM. This does nothing by default but can be turned on with a config vt-kam-allowed
  • Fix bug splitting multi-cell chars in insert mode, also insert blanks CSI
  • Document SRM, but do not implement since no terminal implements this anymore.

This has an associated config `vt-kam-allowed` which defaults to "false"
since this mode can cause the terminal to become mostly unusable. We
include this mode for completions sake however.
