One of the action items of the COE on hyper's surprise CVE is to convert the existing bug report template from markdown to a form. This page may help with the conversion: https://docs.github.com/en/communities/using-templates-to-encourage-useful-issues-and-pull-requests/syntax-for-issue-forms

Most of the information in the current template should be carried over and turned into form fields. We may want to add or alter a few, any questions can be resolved in this issue.
