feat(ui): restyle slug field to match v4 design

Summary

Restyles the Slug field component to match the v4 design system. The main change moves from text-based lock/unlock controls to icon buttons positioned inside the input container, consistent with how ReactSelect handles indicators.

Added new LockOpen and Refresh icons with 16px variants. Updated the Lock icon to support both 16px and 24px sizes.

Also added placeholder prop support to the SlugField component and fixed an issue where the error border would incorrectly change to the focus color when clicking into a field with validation errors.
