Update md output rendering of headings #4380

As discussed in #4286 (comment)

Let's update md output so that it renders as follows:

## title (id)

## only title

## only id

## custom rendering if neither title nor id are present.

Custom rendering is outside of the scope of this issue and to be handled explicitly by commands that need it to show meaningful heading on each item.
