Course/Transcript Evaluation Eligibility
The Undergraduate Prerequisite Course Evaluation is available for students to use before applying to the Prelicensure, RN-BS, or Express Pathway programs. Students are not required to complete an evaluation before applying. This evaluation is intended for students who would like to confirm whether their courses meet the prerequisite requirements for one of our undergraduate nursing programs.
This is not an official application and will not be considered in any application pool.
What to Submit
In order for us to determine whether your courses meet the content and credit requirements for our prerequisite courses, you must submit a syllabus for each course you would like us to evaluate, along with your official or unofficial transcripts. Audit reports will not be accepted.
You must provide a copy of the official or unofficial transcript from each school where you completed the courses you would like to have evaluated.
If you have not yet taken a course, you may still request an evaluation. In this case, you do not need to submit a transcript for that course. However, you must still provide the course information listed below, along with the required syllabus or official course description.
Transfer credit listed on a transcript does not eliminate the need to provide a transcript from the original institution. For example, if your University of Utah transcript shows transfer credit from Salt Lake Community College (SLCC), you must still provide an unofficial SLCC transcript.
Courses Taken Outside the United States
If you completed prerequisite courses outside the United States, you must provide all three of the following:
- Official English translation of your original transcripts.
- WES or NACES credential evaluation showing the U.S. equivalency of your coursework. This is important because we need to verify that the courses meet our requirements for credits and grades.

- Syllabus for each course you would like to have evaluated. The syllabus must be provided in English.

Transcript Requirements
- Transcripts must be uploaded as PDF files.
- Each file must be 4 MB or smaller.
Clearly Identify the Courses You Want Evaluated
Please provide a list of the courses you would like to have evaluated. For each course, include:
- Institution where you completed the course
- Course name and course number
- Grade earned
- Number of credits
- Term completed: Semester and year (e.g., Fall 2025)
Example: University of Wisconsin – MATH 321: Introduction to Statistics – 3.0 credits - Fall 2025
Course Syllabi and Course Descriptions
Please provide a syllabus for each course you would like us to evaluate. If a syllabus is not available, please provide a link to the official course description for the course.
During the evaluation, we review the syllabus to determine whether the topics and course content align with our prerequisite requirements. We also verify that the course meets our standards for credits, grade, and school accreditation.

Please note: If your course evaluation request does not include the required information and documentation listed above, your request will not be processed.
We only evaluate prerequisite courses required for our nursing programs. We do not evaluate general education courses.
