FamCare: how we designed AI into a family health-coordination app
The problem- A healthcare adherence crisis hiding in plain sight. India treats adherence as the patient's problem, when, for chronic conditions, it's really a family coordination problem. 84% of hypertensive Indians don't take medication as prescribed. India's 60+ population is doubling to 347M by 2050, with ~70% depending on family for daily care. Yet tracking doses and chasing refills falls on one person, managed manually, every day. No hospital app or EHR solves that. That's the gap FamCare targets.
How AI is woven into the product:
1. OCR onboarding- scan a medicine strip, auto-extract dosage. Under 90 seconds to first reminder. AI removes effort at the point of entry.
2. Predictive refill- reasons over consumption pace + delivery lead time to alert before a pack runs out, not on a fixed calendar. This is the product's "aha" moment.
3. A relationship graph, not a database- care activity modelled as who's caring for whom, deepened by real family data over time. A reminder UI can be copied in a sprint; this can't.
4. Intelligence is Phase 3, not Phase 1- validate the coordination loop first, build the care graph next, add the AI Care Assistant only once there's real consented data to ground it.
5. Compliance shaped the AI, not the reverse- scoped against DPDP/ABDM; no diagnosis chatbot, no data monetisation, until legally cleared.
The AI product management layer underneath:
Gate AI on data readiness, not model readiness
Design the metric before the model- our North Star (doses tracked-and-confirmed per family, per week) is built so a single-user account can't inflate engagement
Validation gates- SUS 68+, PMF ≥40%, before each phase unlocks
Guardrails before capability- compliance and scope locked before the feature list
The real lesson: the hard AI product decision isn't "what can we automate"- it's "what should we wait to automate until we've earned the data and the user's trust." In healthcare, that discipline is the product.
